Joint Commission's Office of Quality Monitoring

You are encouraged to report any conditions or issues that you feel could impact the quality of care or safety of patients being treated at LADHS facilities to the LADHS Patient Safety Hotline at (213) 989-SAFE (7233) or (800) 611-4365.

The public may also contact the Joint Commission's Office of Quality Monitoring to report any concerns or register complaints about a Joint Commission-accredited health care organization by either calling (800)-994-6610 or e-mailing complaint@jcaho.org .

REPORTING PATIENT CARE OR SAFETY CONCERNS

You may report any patient care or safety concerns as follows:
